Youth Development offers a range of activities, workshops and events for young people aged 11 - 25

Events will have different age restrictions

Please complete one registration form for each young person.

Places are limited so bookings are essential.

In the event of any accident or illness suffered by myself or my child I authorise the organisers of this program to obtain on my behalf, any medical assistance required and I agree to reimburse the organisers for any expenses incurred.

I understand that the City of Mandurah aims to create a safe space for all young people.

I acknowledge that if my or my child's behavior is deemed to be disrespectful of the people and the place, they will be asked to leave the program. Parents/Guardians will be contacted to arrange to collect their child or to arrange transport.

I have disclosed any behavioural / medical concerns that may impact on the safety and wellbeing of the participant, other young people or staff at the activity.

I understand that staff may contact me should any potential risk to the participant or others be identified.
